You may use this form to request an appointment. Your name, phone number, email address, and a brief explanation (message) are required fields. I will do my best to get back to you (via email) within 48 hours of receiving your request. At this time, appointments cannot be scheduled online; however, the New Client Forms are available HERE.

 

FAQs:

Do you take my insurance?

- I do not do ANY business with insurance companies. My practice is private pay (self-pay) only. I do not utilize any insurance codes or forms. There are multiple reasons, which I would be happy to discuss if you are interested. 

 

Do you do late or weekend appointments?

- I see clients between 10AM and 5PM Monday through Thursday.

- Weekend intensives may be an option in the near future.

Do you offer in-person appointments?

- My practice is a hybrid practice, meaning I offer both in-person and virtual appointments. My office is located in Lake Charles, Louisiana. I utilize a secure, online teletherapy platform, which can be accessed from any internet-capable device and in the comfort of your own space.
